POSITION DESCRIPTION: BRIDGES operates a variety of programs guided by the Philosophy of Independent Living. The Peer Advocate is essential to providing advocacy, support and assistance for individuals in our senior and disability community in Rockland County.

 

QUALIFICATIONS:

General

· An Associate’s Degree preferred and/or equivalent work life experience in a similar capacity with the population served.

· Excellent interpersonal, organizational, written and oral communication skills required; able to interact in a respectful, supportive and confidential manner

· Strong familiarity with Independent Living Philosophy, support for the senior and disability community

· A valid NY State Driver’s License and driving record acceptable for agency insurance coverage are required

· Remain current in all agency required trainings

· Maintain strong work ethics, and strict adherence to code of ethics, standards of conduct and confidentiality requirements

· Self-motivator and ability to work independently

· Capable of communicating in a manner that is culturally sensitive to individuals with disabilities

· Must be able to travel locally for meetings, home visits or events

· Working knowledge of Microsoft Office Software, including Excel, Word and Power Point. Working knowledge of Gmail/Google applications

· Ability to work independently in an efficient manner

DUTIES & RESPONSIBILIITIES

General

· Conducts Information & Referral as well as Service Records for each person served

· Complies with federal, state, and local financial legal requirements by studying existing and new legislation, enforcing adherence to requirements, and advising management on needed actions

· The Peer Advocate functions as a model to support individuals we serve

· Provides education on how employment effects any benefits the individual is receiving

· Provides advocacy for individuals facing obstacles, barriers, or accessibility issues at the workplace and/or housing

· Provides peer counseling and independent living skills training

· Connects people with supportive services available through BRIDGES or the community

· Maintains strict confidentiality and protects Agency operations by keeping information confidential and secured

· Adheres to all Agency policies and procedures, including but not limited to corporate compliance, personnel and fiscal policies and procedures

· Provides monthly reporting as required

· Offer support and guidance for residents to maintain their best quality of life

· Identify barriers (internal and external) to full participation in community resources and developing strategies to overcome those barriers.

· Conduct home or office visits as needed for residents to offer assistance and support

· Maintain accurate record keeping, note taking, and data entry
